R.I.S.E. pairs short, non-diagnostic assessments with coaching so you, your child, and their teacher get a clear picture of how your child learns and a plan to act on it. Pick what fits below and watch your total add up as you go.

Coaching works as a team. Your child, you, and the teacher each have a part, and progress depends on the practice that happens at home. R.I.S.E. does not “fix” children or promise miracles — it helps them with parent and teacher support.
No services begin until your first payment is received. For Step Up families, services begin when your scholarship funds are confirmed.

1 About you and your child

Schools and homeschool groups enrolling a class or group: use the school and group enrollment form for group pricing.

2 Choose your package

Add-on assessments — $125.00 each

Add one or more to your package. Each includes administration, scoring, a written summary, and review with you. In Package B, an add-on is re-checked at the 90-day re-profile at no extra charge. The reading and math screeners are informal coaching tools; they do not diagnose dyslexia, dyscalculia, or any other condition.

À la carte services

Paid in advance when you book. Payment plans and the pay-in-full discount do not apply.

3 How would you like to pay?

Payments are due on the dates shown in your total. Each payment has a 2-day grace period. After that, a one-time $25 late fee applies to that payment and services pause until it is paid. If the payment is still not received 7 days after R.I.S.E. sends you written notice, R.I.S.E. may end the agreement. If that happens, you pay only for services already delivered plus any late fee owed, and any unused prepaid coaching sessions are refunded under clause 10.

What is going on? (optional)

This section is for Dr. LeLi’s awareness, so coaching can be planned safely and respectfully. R.I.S.E. reports and coaching do not diagnose any condition and do not prescribe, recommend, or change any medication. They do not replace care from your child’s doctor or other licensed providers. What you share here is kept private, is seen only by Dr. LeLi, and is never shared with a teacher or school without your written permission.

5 Agreement and permission

Please read the service agreement below, initial the four key points, and sign by typing your full name.

Program totals (your choice)

May R.I.S.E. count your child’s results in combined totals with all names and identifying details removed? For example: “8 out of 10 students in our program improved their planning skills.” These totals help R.I.S.E. improve its programs and apply for grants that can lower costs for families. No one could tell the totals include your child. Your answer does not change your child’s services in any way, and you can change it later by email.
