This profile is an educational self-report used for transition coaching only. It is not a VA disability evaluation, a psychological evaluation, or a screen for PTSD, TBI, depression, or any medical or mental health condition. If completing it brings up a persistent lack of safety, thoughts of self-harm, or a mental health crisis, please stop and contact the Veterans Crisis Line — dial 988, then press 1 (or text 838255).

Once you enroll in the Veteran & Military-Connected Learner Track and payment (or an approved payment plan) is in place, the assessment unlocks in your client portal. A R.I.S.E. practitioner scores your profile and builds your transition roadmap with you.
